2. Fill out the basic forms and have them signed from the principal researcher and the head of the department - the academic supervisor in the case of the principal researcher from undergraduate or postgraduate students. (Attached are the updated forms approved by the committee).

3. The researcher must attach the certificate of passing the electronic course with the supporting documents for the application (such as informed consent, the research form and the questionnaire).

4. The researcher must attach an updated CV

5. If the researcher is a bachelor's or postgraduate student, a letter from the academic supervisor must be attached.

6. If the sample is a minor (under the age of 18), a non-adult patient consent form must be used.
